Requirements & Qualifications:

• A valid NJ CHHA certificate
• Picture Identification Card
• Social Security Number
• Pass Basic Exam
• Pass a Drug Screen Test
• Medical Exam with the year (2 PPD or Quanteferon Test, History of Positive PPD with Chest X-ray)
• Immunization Lab Report ( MMR)
• Proof Of COVID Vaccination Card
